The community nursing and therapy team had undergone substantial change. The new system was not coping with the demand being placed on it. WLP were requested to understand the causes and opportunities to resolve.
Our consultant Mapped the system and analysed SystmOne data to understand demand and capacity. Clinicians and GPs were interviewed and surveyed to understand patient needs and improvements were recommended.
The client has implemented many of the recommendations made to them. As a result, capacity, continuity of care and GP satisfaction has improved.
